YP ACP Image.pngWhatever stage you’re at in your life, it can be useful to plan for the future. The resources below will help you understand what Advance Care Planning is, along with tools to help you think about the future. A Recommended Summary Plan for Emergency Care and Treatment (ReSPECT) is used in Adults’ services in Herefordshire and Worcestershire as a way of planning for the future and communicating your wishes. It is a personal choice for each young person as to how they want to be involved in this process and some young people may not want to be involved at all.

You may have already discussed and recorded your wishes for future care in a Paediatric Advance Care Plan (ACP).  As part of transition, you may be asked to review your current plan and record your wishes using adult advance care plan or ReSPECT plan.  Using adult ACP documents will enable your preferences to be shared with the adult professionals providing your care.
