Advance Care Planning (ACP) is a voluntary process of person-centred discussion between an individual and their care provider(s) about their preferences and priorities for their future care. It is a personal choice for each young person as to how they want to be involved in this process and some young people may not want to be involved at all.
A young person may reach the end of their life at any point along their transition journey or in early adulthood. Research suggests that creating an ACP can help young people and their loved ones feel more confident that their care and treatment will be focussed on what matters most to them in a personalised, holistic way and helps them to live as well as possible.
A Recommended Summary Plan for Emergency Care and Treatment (ReSPECT) process creates personalised recommendations for a person’s clinical care in emergency situations when they may not be able to make decisions or express their wishes.
For children and young people under the age of 18, preferences for care may be documented in a Paediatric Advance Care Plan (ACP) and for those aged 18 and over, in a ReSPECT Plan or Advanced Care Plan. Children’s services will discuss changing from a Paediatric ACP to an adult ReSPECT Plan or ACP with young people and their family/carers as part of transition.
