The transition journey for you and the young person you care for, may involve moving on from the services provided by a children’s hospice. Below are links to hospice and specialist palliative care services provided across Herefordshire and Worcestershire. There is one children’s hospice serving Herefordshire and Worcestershire (based in Worcester) and four adult hospices across the two counties.
Hospice UK is the national charity for Hospice and end of life care and has information for professionals, patients and their loved ones and carers.
Acorn's Children's Hospice
Acorns provides a holistic range of services for children and young people up to the age of 18 who are life limited, and life threatened. The services are available 24 hours a day, 365 days each year across our three hospice sites in Birmingham, Walsall and Worcester. Acorns for the Three Counties, based in Worcester, supports families from Worcestershire, Herefordshire, Gloucestershire and South Warwickshire.
At Acorns we provide specialist care that is tailored to the child and family’s specific needs; we support the whole family as and when they need it, throughout their journey from referral to bereavement or transition into adult services. When a young person reaches the age of 14, we provide information to support the decisions the young person and their family may have to make about adult care. Working with other professionals, the young people and their families, we support their transfer from Acorns to the appropriate adult services by the time they are 18.
Referrals are accepted from a pediatrician, nurse or midwife. If you would like your child or someone you care for to be considered for referral speak to a trusted health care proffessional. More information on Acorn's referral process and acceptance criteria can be found on their website, here.
