At risk of Type 2 Diabetes?
If you are pre-diabetic, or have a history of gestastional diabetes, the Healthier You National Diabetes Programme might be for you. You can sign up without a referral from a GP. To find out more, see the Healthier You section below.
Do you know your risk of type 2 diabetes? Use the Know Your Risk tool to find out! You will need a measuring tape for your waist size.
If you score 16 or higher, you may be at risk of developing type 2 diabetes. Contact your GP practice about having a blood test and to check if you can join the Healthier You National Diabetes Prevention Programme.

The Healthier You NHS National Diabetes Prevention Programme is a free 9-month programme for people with either:

Pre-diabetes
-
A history of gestational diabetes.
It can help you to make lifestyle changes to reduce your risk of Type 2 Diabetes. Health coaches will help you to create a personal plan and give regular support throughout your journey.
Choose to from two routes in the programme
Group sessions: Join a small group of people also at risk of developing type 2 diabetes. You are welcome to take someone with you for extra support. There are 13 sessions spread over nine months, hosted by your Health Coach.
Digital: Get support from a health coach over 9 months through your smartphone. Using the Liva app, you can track your personal health goals, get advice, connect with others on the programme.
Prediabetes means that your blood sugars are higher than usual, but not high enough for you to be diagnosed with type 2 diabetes. It also means that you are at high risk of developing type 2 diabetes.

Healthier You
Completing the Healthier You (NDPP) can help you reduce your risk of type 2 diabetes.
To register, you will need a blood test result from within the last 12 months showing you are pre-diabetic.
If you have had a blood test within the last 12 months, you can check your results letter or contact your GP practice to ask for the results.

Gestational Diabetes Mellitus (GDM) is when you have high blood sugars (glucose) during pregnancy that returns to normal once pregnancy ends. Up to half of women diagnosed with GDM go on to develop type 2 diabetes within 5 years.
Reduce your risk of type 2 diabetes and join the Healthier You NHS Diabetes Prevention Programme today! It’s quick and easy to register online. You can also speak to your midwife or GP practice to help you.
