The LDA Admission Avoidance and Discharge Team works across Health, Education and Social Care systems, alongside identified Partners who support the programme delivery, with coproduction and development to support the implementation of the policy.
The LDA-AAD team provide the following roles and responsibilities to ensure the policy is delivered and embedded into our local systems, to meet the national trajectories around specialist in-patient bed usage, to inform the commissioning intensions across our local systems, that support the development and review of community services.
